As with accidental training deaths, LE needs to recognize that "friendly fire" tragedies are both needless and preventable. And the only acceptable goal in any/all accidental police deaths is their total elimination.

Command staff members are now discussing new rules governing plainclothes work. Officers could wear distinctive colors that change by the day or week, to make them quickly identifiable to fellow officers but not to the people they are trying to arrest, or use code words.

Two NYPD Officers Shot In Harlem Gunfight at 500-Person Barbecue
Angel Alvarez, 23, shot an killed 21-year-old Luis Soto, then exchanged fire with at least five officers. Alvarez fired at least four rounds, and Officer Michael Tadeschi was likely struck by a friendly fire round that deflected after striking his ballistic vest.
