N.C. SWAT Officer Killed While Re-Inserting Flash-Bang Pin

Charlotte-Mecklenburg Police SWAT veteran Fred Thornton died after incorrectly attempting to re-insert the pin into a flash-bang device when it exploded, according to preliminary results of the investigation.
